The primary goal of this role is to increase awareness of Health-Related Social Needs (HRSN) services in Oregon and promote Medically Tailored Meals (MTM) from Mom’s Meals across the state. HRSN Services are a part of the 1115 Demonstration Waiver that has allowed eligible Oregon Health Plan members to access services related to nutrition, housing, and climate. The ideal candidate will be able to clearly articulate the importance of nutrition in supporting positive health outcomes, especially for individuals managing chronic conditions, experiencing food insecurity, or facing other social determinant of health-related challenges. A strong passion for helping Medicaid beneficiaries access services that can improve their quality of life is essential.
This position requires the ability to build and maintain a well-organized network of contacts and to identify organizations that serve eligible populations. The candidate will actively participate in webinars hosted by Coordinated Care Organizations (CCOs), county agencies, and statewide partners to stay informed on best practices, program updates, and future engagement opportunities. They will also frequently Mom’s Meals at in-person community events such as health fairs and resource fairs.
To drive MTM referrals, the candidate will educate organizations through phone calls, virtual meetings, and in-person networking. Success in this role depends on building strong, trusting relationships with a wide range of partners, including Community-Based Organizations, housing and rental assistance providers, healthcare and behavioral health providers, Community Health Workers (CHWs), Federally Qualified Health Centers (FQHCs), clinics, hospitals and health systems, CCOs, outreach and engagement teams, and HRSN service providers.
Ideally, this position will be based in the Portland Area. It will also be eligible for a company car.
Salary range: $60,000-80,312
